These Terms apply to your use of the Chronic Doctors system and services.
1. Nature of Services
Chronic Doctors provides a patient-facing platform facilitating access to telehealth consultations delivered by independent registered practitioners.
We do not provide medical advice or treatment directly.
2. Consent to Collection and Use of Information
We collect personal and health information necessary to:
provide medical care, treatment and advice
manage appointments, billing and administration
comply with legal and regulatory obligations
This may include:
identity and demographic information
health and medical history
consultation records, prescriptions and communications generated via practitioner systems and the platform interface
platform usage data including login activity and audit logs
You acknowledge that failure to provide accurate information may impact care.
3. Consent to Telehealth
By proceeding, you consent to:
receiving healthcare services via telehealth
your information being collected, used and disclosed for care and operational purposes
your medical records being created and maintained by your treating practitioner within internal Practice Management Software (PMS) systems
You understand:
telehealth has limitations compared to in-person care
clinical decisions rely on information available remotely
4. Platform Structure and Data Access
You acknowledge that:
Chronic Doctors provides a patient-facing platform
practitioners utilise internal PMS systems to manage clinical records
patients do not have direct access to the PMS
practitioners are responsible for clinical records and act as data custodians
Access to systems is restricted to authorised users and governed by role-based permissions.
System activity, including access to records, is logged via audit logs.
We may restrict, modify or revoke system access where required to maintain system integrity, security or compliance.
5. Use and Disclosure of Information
Your information may be used and disclosed to:
treating practitioners
healthcare providers involved in your care
pharmacies, Medicare, DVA and insurers
service providers supporting the platform and PMS systems
regulators where required
Information may also be used for:
quality improvement and practice management
training, education and research (de-identified where appropriate)
6. Communication Consent
You consent to being contacted via:
phone
SMS
email
electronic messaging
You acknowledge that electronic communication may not always be secure.
7. Risks and Limitations
You understand:
technical failures may occur
consultations may be interrupted or delayed
outcomes may differ from in-person care
For urgent medical issues, you must seek immediate care.
8. Prescribing and Treatment
You acknowledge:
prescribing is at the sole discretion of the practitioner
not all treatments are suitable via telehealth
additional verification or in-person consultation may be required
9. User Obligations
You agree to:
provide accurate information
maintain confidentiality of login credentials
not misuse the platform
10. Fees and Payments
You agree:
fees apply to services
payment is required via approved methods
refunds are subject to Australian Consumer Law
Missed appointments and late cancellations may incur fees.
11. Acceptable Use
You must not:
impersonate another person
use the platform unlawfully
interfere with system security or performance
12. Limitation of Liability
To the extent permitted by law:
practitioners are responsible for clinical care
Chronic Doctors is not liable for medical outcomes
liability is limited to platform services
Nothing excludes rights under Australian Consumer Law.
13. System Features
Any platform tools:
are informational only
do not constitute medical advice
must not be relied on without practitioner guidance
14. Indemnity
You indemnify us against loss arising from:
misuse of the platform
breach of these Terms
unlawful activity
15. Termination
We may suspend or terminate access where:
Terms are breached
system security or integrity is at risk
16. General
We may update these Terms from time to time. Continued use constitutes acceptance.
These Terms are governed by the laws of Queensland, Australia.
